How to file a claim

Starting your claim only takes a few minutes. 

Be sure to have the following information handy before getting started: 

  • Policy Number
  • Policyholder Contact Information
  • Injured/Sick Information (if different than Policyholder)
  • Date of Accident/Sickness
  • Brief Description of Accident/Sickness

You will also be asked to submit relevant documentation related to the injury/sickness.  Depending on the coverage you have, this may include doctor notes, medical receipts and team registration information. You can file your claim online here.

To ensure your child’s claim is processed without delay, please follow these steps: 

  1. Fill out our initial claim intake form here.  

You will receive a confirmation email from our third-party claims handling team, Co-Ordinated Benefits Plans. To proceed, complete their claim form and submit it within 90 days of the injury. Mail to: Co-ordinated Benefit Plans, PO Box 21282, Tampa, FL 33622 or email to: [email protected]

  1. Send any medical bills to your primary health insurance first if your provider hasn’t already done so on your behalf. They will process your claims then we will review the outstanding balance.  We’ll help cover eligible medical and dental expenses that aren’t paid by your primary health insurance. 
  2. Once your primary health insurance has processed your medical bills and provided an Explanation of Benefits (EOB), please send us the EOB along with copies of the original medical bills. Request your provider issue bills using either Form UB-04 or Form CMS-1500. Then, submit these documents to our third-party claims handling team. Please note: All expenses must be incurred within 52 weeks (1 year) of the injury.
