How to file a claim
Starting your claim only takes a few minutes.
Be sure to have the following information handy before getting started:
- Policy Number
- Policyholder Contact Information
- Injured/Sick Information (if different than Policyholder)
- Date of Accident/Sickness
- Brief Description of Accident/Sickness
You will also be asked to submit relevant documentation related to the injury/sickness. Depending on the coverage you have, this may include doctor notes, medical receipts and team registration information. You can file your claim online here.
